2. Medical Services & No Guarantee of Results
Our services are provided by or under the supervision of licensed medical providers and are delivered in person at our Maple Grove, Minnesota location. All care begins with an evaluation, and treatment is provided only when a provider determines it is appropriate. We do not guarantee any specific result. Please review our Medical Disclaimer. This website does not create a provider–patient relationship; that relationship is formed only through an in-person evaluation.
3. Appointments, Cancellations & Deposits
Appointments are subject to availability. Cancellation, rescheduling, no-show, and deposit terms are described in our Cancellation Policy.
4. Pricing, Payment, Memberships & Refunds
Prices are provided at the time of consultation and may change. Consultation, program, membership, and product fees are described to you before you incur them. Memberships. Because our services are medical and personalized, fees for services rendered are non-refundable; unused prepaid packages may be refunded or credited per the terms disclosed at purchase. We do not sell prescription medication as a retail product; medications are dispensed only pursuant to a valid prescription.
5. Communications
By providing your contact information, you consent to receive communications from us about your appointments and care. Marketing communications are optional; you may opt out at any time, and opting out of marketing will not affect your care. Handling of your information is described in our Privacy Policy and, for health information, our Notice of Privacy Practices.
6. Intellectual Property
