Does Testosterone Therapy Affect Fertility?

Testosterone Therapy for Fertility

What Every Man Should Know First

Yes. Testosterone therapy can lower or even stop sperm production. It’s one of the most important facts about TRT, and one of the least discussed in online testosterone ads. If you want children now or later, tell your provider before you start.

Why added testosterone lowers sperm count

It sounds backward, but it follows from how the body regulates hormones. Your brain releases signaling hormones (LH and FSH) that tell the testes to make testosterone and sperm. When testosterone comes from outside the body, the brain senses there’s plenty and turns those signals down. Testosterone in the blood stays up, but sperm production inside the testes can drop sharply.

What this means for you

  • Trying to conceive now: Testosterone therapy is usually not the right starting point. Your provider will discuss other approaches and may coordinate with a fertility specialist or urologist.
  • Want kids in the next few years: Say so at your first visit. Fertility-preserving options can be discussed before treatment starts.
  • Done having children: The fertility effect matters less, but monitoring for other markers still applies.

Is the effect permanent?

For many men, sperm production recovers after stopping testosterone, but recovery can take months and isn’t guaranteed. Length of treatment, age and baseline fertility all play a role. This is exactly why the conversation belongs before your first dose, not after.

Low testosterone can affect fertility too

Low T itself is sometimes linked to reduced sperm quality and lower libido, which can make conceiving harder. That’s why a full evaluation looks at the whole picture: hormones, general health, weight, sleep and goals. See what a men’s health blood panel checks.

How Remedy handles fertility in men’s hormone care

At our Maple Grove clinic, family planning is part of the first conversation with Samantha Youngs, PA-C. Your plan is built on your labs and your life stage, and we’ll tell you plainly if a therapy isn’t the right call.

FAQ

Can I use testosterone as birth control?

No. Sperm suppression is unpredictable and testosterone is not a contraceptive.

Do BHRT pellets affect fertility the same way?

Pellets deliver testosterone too, so the same concern applies. Compare delivery methods in injections vs. pellets.

Should I bank sperm before starting TRT?

Some men choose to. It’s worth discussing with your provider, especially if your plans are uncertain.
